A “Good Faith Exam” medical clearance questionnaire is required for all patients prior to receiving treatment. This is a standard policy with no exceptions. Instructions for completing the medical clearance will be provided when you book your appointment.
Insurance:
Canvas By N does not participate with or submit billing to private insurance companies. However, we are happy to provide you with a copy of your invoice upon request so that you can submit it directly to your insurance company for reimbursement.
Payment/Deposit:
Full payment is due at the time of service. To secure your appointment, a non-refundable $100 deposit is required in advance for all services. This deposit will be applied toward your treatment on the scheduled date. If no treatment is performed, the deposit will be considered a consultation fee. Please note that this deposit is non-transferable and cannot be applied to future appointments if you complete a consultation and choose to schedule treatment at a later date. In such cases, the deposit will be considered a consultation fee, and a new deposit will be required for any subsequent appointments.
Forms of Payment:
We accept payments via cash, Visa, MasterCard, American Express, and Discover. Please note that all payments are the patient’s responsibility, and all sales are final.
Payment Plans:
We offer payment plans. Please contact our office at (818) 600-8126 to inquire about eligibility.
Refund Policy:
All sales are final. We encourage you to carefully review all required protocols and potential side effects before proceeding with treatment. If you have any questions or concerns, please feel free to reach out before purchasing, as we are here to ensure you are fully informed.
Package Agreement:
All packages are non-refundable, non-transferable, and cannot be substituted for any other packages once purchased.
Service Policy:
Canvas By N reserves the right to refuse treatment or discontinue service at any time. The medical provider will determine whether you are a candidate for any particular service.
Scheduling:
A $100 deposit is required to secure your appointment.
Rescheduling/Cancellation Policy (Past 72 Hours):
We have a 72-hour window for rescheduling and cancellations. We understand that unexpected situations can arise; however, we kindly request that you provide adequate notice to respect both the provider’s time and the opportunity for other patients to utilize the appointment slot. You will receive appointment reminders 5 days prior, 3 days prior, and on the day of service. Please feel free to reach out during business hours with any questions. Once the 72-hour window closes, the deposit becomes non-refundable or transferable.
Rescheduling/Cancellation Policy (Within 72 Hours):
If you cancel or reschedule within the 72-hour window, your deposit may be applied as a credit toward a future appointment, but only once. This credit must be used within 6 months, after which it will be forfeited.
No Shows:
If a patient fails to show up for their scheduled appointment, Canvas By N reserves the right to refuse service for future appointments. We take this step to ensure fairness and respect for all clients, as missed appointments prevent others from receiving care. We ask for your understanding and cooperation to maintain the integrity of our scheduling system.
Late Policy:
We understand that LA traffic can be unpredictable. If you are running late, please call our office. We offer a 15-minute grace period. If you arrive more than 15 minutes late, we may need to reschedule your appointment to respect the time of other patients. This policy helps us ensure optimal care for all our clients.
