1. Insurance: Canvas By N does NOT participate with or submit billing for any private insurance companies. However, we can provide you with a copy of your invoice(s) upon request so that you are able to complete the appropriate forms for patient reimbursement from your insurance company.

2. Payment/Deposit: ALL payment is expected at the time of service; In order to secure all future appointments up to date card information is required on file, ALL services require a $100 non-refundable deposit in advance. This deposit will be used towards the treatment of choice on the scheduled appointment date. 

3. Canvas By N only accepts payment in the form of cash, check, VISA, MasterCard, American Express, and Discover.

4. Refund Policy: ALL SALES ARE FINAL. Before a service is performed please consider all the required protocols and side effects. We are committed to client satisfaction and are available to answer any questions or concerns you may have in regards to the services we offer before purchase.

5. Package Agreement: All injectables, facials, and/or any other custom packages are all NON-REFUNDABLE and cannot be substituted for any other packages.

6. Services Policy: I understand Canvas By N has the right to refuse treatment and/or dismiss a client from any service at any time. I also understand that I may not be a candidate for certain medical services and it is at the full discretion of the medical provider to determine whether I am a candidate for any service provided.

I, the patient, acknowledge that to confirm my appointment slot, it is necessary to maintain a valid card on file at all times. I authorize With Canvas By N facility to charge a non-refundable deposit of $100.00, which can be utilized for my treatment or as a consultation fee during my official in-office appointment. Must consent to the facility retaining my card information on file.

You, as the patient, understand that I am permitted to reschedule my initial appointment only ONCE within the 72-hour rescheduling policy window, utilizing the initial $100 deposit. You understand that any subsequent rescheduling attempts, whether within or beyond the 72-hour rescheduling timeframe and regardless of unforeseen circumstances, will require the submission of a NEW deposit and the scheduling of a NEW appointment. I further understand that once I have rescheduled my appointment once, as stipulated within this consent, my initial deposit becomes non-refundable.

To request a reschedule, clients should contact our office (818) 600-8126 during business hours 72 hours before the scheduled appointment time

You as  the patient, understand that notification of appointment cancellation given within 72hrs from when the appointment was made, is eligible for the $100 deposit refund. If appointment is canceled after 72hrs, the $100 deposit is nonrefundable. It MAY be applied towards deposit credit for future appointments re-scheduled ONLY ONCE, if in the case you are given the opportunity to transfer you have only 6 months within the year to use the deposit. After 6 months your deposit will no longer be valid.
